What you need to register:

1.      Complete Registration Form 

2.      A copy of your child's birth certificate (Not required for returning players)

a.   If you do not have the Birth Certificate, this link can help you get one.  It must be turned into the league by by July 15, 2008.

3.      The registration fee.

a.     $110 for Tiny Mite football or Mascots for cheer

b.     $210 for all others

                                                              i.      Scholarships for up to 50% of the fee are available - eligibility is the same for qualifying for free/reduced lunch at school, so bring that letter as proof of eligibility if this applies.

4.      Health insurance information (name of provider, the group ID number, and the individual's ID number) and a completed medical history form.

5.      Each family will need to donate 10 hours of volunteer time.  You may opt out of volunteering at a fee of $195.     Volunteer Form only available at registration right now. If you wish you to coach, fill out out this the Coaches Application.
 

6.      All parents that will be volunteering will need to fill out a form and provide a copy of their driver's license for a background check.    

a.      This background check is run through a background check service and there is no fee required from you.

7.   Submit a Code of Conduct form
 

8.      Anyone wanting to be a team parent, player administrator, a head coach  or assistant coach must submit $10 and a form for an Oregon State Police background check.

9.      Your child's weight and photo will be taken at registration. Cheer participants will be fitted at registration.  Football fittings will happen at certification.

OTHER - Due prior to practice on August 18.

10.      Sports Physical: A copy of your child's sports physical. Due to League ASAP Player can not participate until sports physical is completed.

a.      Submit at registration if already done in 2008, otherwise it is required to be turned in before your child may participate.

c.      You can schedule a physical with your physician, or you can bring
your child to one of the sports physical clinics.  There will be info on the www.LBPopWarner.com Web site with dates and times. Some parents have told us Urgent Care does sports physicals, the cost is around $75.

11. Report Cards from last reporting period: Due to Coach prior to first practice in August.
