Scholarships for up

50% of the fee are available - eligibility is the same for qualifying for
free/reduced lunch at school, so bring that letter as proof of eligibility
if this applies.
4. Health insurance information (name of provider, the group ID number, and the individual's ID number) and a completed medical history form.

Each family will
need to donate 10 hours of volunteer time. You may opt out of volunteering at a
fee of $195.

Coaches Application. 6. All parents that will be volunteering will need to fill out a form and provide a copy of their driver's license for a background check. a. This background check is run through a background check service and there is no fee required from you.
7. Submit a
Code of Conduct form 8. Anyone wanting to be a team parent, player administrator, a head coach or assistant coach must submit $10 and a form for an Oregon State Police background check. 9. Your child's weight and photo will be taken at registration. Cheer participants will be fitted at registration. Football fittings will happen at certification. OTHER - Due prior to practice on August 18. 10. Sports Physical: A copy of your child's sports physical. Due to League ASAP Player can not participate until sports physical is completed. a. Submit at registration if already done in 2008, otherwise it is required to be turned in before your child may participate.
